Hey everybody,

We’re looking for ways to improve youth group, and we’d love your input! We designed a survey that is super quick and totally confidential.

We can’t see who the responses are from, and we’re not going to have our feelings hurt, so please be completely honest. We really value your opinions. Also, if there’s anything we didn’t ask that you’d like to say, go for it!
